What to Expect on your First Visit?
As a new patient, you will receive a thorough examination of your areas of complaint. Our Doctors will also evaluate your range of motion, muscle strength and neurological sensations that relate to your spinal column and exiting nerves. Your spine will be examined in order to see if you have any vertebral subluxations or misalignments. We will take any needed x-rays in order to better visualize the biomechanical state of your spine.
If you have recently had an MRI, CT scan or x-rays taken at another facility, please bring copies of the films or reports with you. After your exam, the Doctors direct the therapists on where to begin pin-pointing the source of your pain or discomfort. You then receive neuro-muscular trigger point therapy, which will release tension on the spine and release toxins from the muscles, making spinal adjustment easier, perhaps with little or no force. After review of your exam, x-ray and diagnostic testing, you may be assessed for the need for physical therapy. On your second visit, you will be given a complete Report of Our Findings and a personalized plan for treatment.

Please arrive for your appointment 15 minutes prior to the scheduled starting time in order to fill-out appropriate paperwork.
Online Patient Information Forms
In order to better serve you, the New patient Packet is now available online. Included is each form required when a patient visits our office for the first time. You may print these forms, fill them out and bring them in on the day of your appointment or prior to the date of your first appointment. Remember, if you have seen another doctor for the condition you are seeking care from Trinity Pain Relief Center for, be sure to have any test results, imaging results (xray, MRI or CT scan), or surgical notes forwarded to our office prior to your first visit (see Records Transfer/Request Form in New Patient Packet).

Potential patient, please understand that PRIOR to the first office visit, you should fill this form out and send it to the doctor’s office you are being referred to us from, or to any doctors office that has test results or xrays/mri reports relating to the condition you will be seeking treatment for. It will speed up your first office visit if we already have any previous important records relating to your condition.
